Creating mouse pointers (or "cursor themes")

I want to be able to create mouse pointers or import Windows-style pointers. As I understand it, these are called "cursor themes". I've been googling like crazy for apps that can do this, but all I've found is icon-slicer, and it doesn't work. So how do you guys create your cursor themes? And can they be converted from Windows?
